Assessment

At eldereval, our assessment is a comprehensive, multi-disciplinary examination with the goal of preparing the client and his or her family to plan for the future by assessing individual needs and strengths. Our home visit for this assessment is unique, since it is our window into the everyday world of each client. We see them in their own environment where they are more relaxed and comfortable among the familiar. We can assess their home for safety and adaptability, and also get a good look at how they manage daily tasks. We will also do a cognitive screen, nutritional assessment, depression scale, medication review, and assessment of their current support system.
We will obtain a health, social, and vocational  history from the client, family members, and clinicians. Family members play a vital role in this informational process.
The assessment process gathers all information possible to use in the Personalized Plan of Care.

Geriatric Consult

Our findings during the home visit and assessment may warrant a consult with a Geriatrician or another physician or clinician. We work with Roland J. Losos, M.D. in Westerly, R.I., and Mystic, Ct., whose bio is included. If you live in another geographic area,  we can recommend an appropriate consultation center or office. The client may also be sent for laboratory tests and/or imaging studies.  These clinicians will give a report to eldereval, and also to the primary physician. Their input will be included in the individualized Care Plan.


Care Plan Development

Following the assessment, eldereval will develop a Care Plan designed specifically to meet the needs of the client. This plan will address specific areas of concern and provide resource information which can assist the client and family. The resources listed in the plan would be appropriate for either maintaining the client at home, or at a facility.This plan will be presented in person if possible. Families may use the Care Plan as a road map for their loved one’s care. If any assistance is needed, they may call for a phone conference at any time. Transitions

Eldereval can assist with any transitional moves from home to a facility, or between facilities. We have extensive knowledge of facilities at all levels of care. We will research and provide a list for you to visit.
Our professional network also includes Senior Move Managers that can assist with sorting, packing, storage and the move itself. Families members that are not local will be updated on the process.

Education and Support

Clients and families will receive pertinent literature, and notice of Community Education programs which would enhance care giving. Eldereval presents several educational programs each year in many areas. There are also support groups which meet monthly. Many are for care partners. There is one group that is for the family and the client together. A complete listing of programs and groups is available upon request.

ROLAND J. LOSOS, MD

Dr. Losos began his medical profession in 1993. He is board certified in Internal and Geriatric Medicine. A graduate of Medical University in Lublindr_losos_accordian_image, Poland, Dr. Losos completed his residency at Wood Hall Medical Center in New York City, an affiliate of New York University, and his fellowship at Our Lady of Mercy Medical Center in Bronx, New York. Dr. Losos is licensed to practice medicine in both Connecticut and Rhode Island.

ROSE LEVINE

Rose is a graduate of the University of Connecticut and the Masters Program in Education at Eastern Connecticut State University. Srose_levinehe also holds a Certificate in the Fundamentals of Geriatrics from the University of Connecticut Health Center. Rose has more than 20 years experience in elder care, both in residential dementia care and as Eastern Regional Director of the Alzheimer’s Association.

LISA RYAN

Lisa has more than 15 years experience in the long term care continuum. She is a certified case manager and a member of the National Association of Professional Geriatric Care Managers. She is also a licensed Nursing Home Administrator in both Connecticut and Massachusetts. Lisa has worked in private practice as a Geriatric Care Manager for several years prior to developing ElderEval and very active in state and local agencies dealing with seniors and the issues of aging.
