Geriatric Care Management

A Geriatric Care Manager is a health care professional with specialized knowledge and expertise regarding issues of aging.

At eldereval, our Care Managers come into the home of your loved one and evaluate their living situation. We assess medical, social, and safety needs, recommend  solutions based on problematic areas,  and coordinate services to be put in place. We will develop a personalized care plan that will maximize independence and well being. Our Care Managers serve as a liaison between out of state family members, physicians, care providers, and community resources.

The use of a Geriatric Care Manager will not only provide you with peace of mind, but other benefits as well. Family members may be uncomfortable with elder decision making, or they may be struggling with family and career responsibilities . Navigating our healthcare system is difficult. Our Care Managers have the expertise and resources to find the best possible solutions for any eldercare issue. Our Care Managers can actually help you avoid costly choices that may have  been a band-aid solution, but are no longer appropriate. Other ways we help our clients contain costs is to identify the possibility of benefit qualification, avoid duplication of services, and un-necessary hospitalizations. We have extensive experience with Long Term Care Insurance.

Our commitment to our clients is to help them remain as independent as possible, living in the setting of their choice. Our services are available as often as you deem necessary, or as needs change. We can visit routinely on a weekly or monthly basis, or when a new issue needs attention.

ROLAND J. LOSOS, MD

Dr. Losos began his medical profession in 1993. He is board certified in Internal and Geriatric Medicine. A graduate of Medical University in Lublindr_losos_accordian_image, Poland, Dr. Losos completed his residency at Wood Hall Medical Center in New York City, an affiliate of New York University, and his fellowship at Our Lady of Mercy Medical Center in Bronx, New York. Dr. Losos is licensed to practice medicine in both Connecticut and Rhode Island.

ROSE LEVINE

Rose is a graduate of the University of Connecticut and the Masters Program in Education at Eastern Connecticut State University. Srose_levinehe also holds a Certificate in the Fundamentals of Geriatrics from the University of Connecticut Health Center. Rose has more than 20 years experience in elder care, both in residential dementia care and as Eastern Regional Director of the Alzheimer’s Association.

LISA RYAN

Lisa has more than 15 years experience in the long term care continuum. She is a certified case manager and a member of the National Association of Professional Geriatric Care Managers. She is also a licensed Nursing Home Administrator in both Connecticut and Massachusetts. Lisa has worked in private practice as a Geriatric Care Manager for several years prior to developing ElderEval and very active in state and local agencies dealing with seniors and the issues of aging.
